Lin Chen has provided a scan of this card hoping I can identify it, but I don't recognise it at all. The card mentions Serie A and shows Kaká, so I presume it was issued during his second spell at Milan, when he re-joined them from Real Madrid in September 2013.
The only things I could find are two photos from a publicity company that worked with ESPN and produced a set of promotional cards that came in a special box (see Kaká in the photos) and includes Cristiano Ronaldo as well.
