The Striker Fact Finder was given away with the third issue of the comic, which was published on 30 January 1970. The Fact Finder includes illustrations of many players including Bill Shankly, John Charles, Kevin Keegan, Steve Bloomer and Trevor Francis.
UPDATE (04-06-2022 10:24):  I always resumed this free gift was included with a comic called Striker, but I've now discovered that Striker was a comic strip in Gary Lineker's Hot-Shot! comic!

Stand-up Footballers weren't a new phenomenon, but this large one of Colin Bell is the only one issued by Striker magazine, which was included with the second issue, dated 23 January, 1970.

This disc was given away with the first ever issue of Striker on 16 January, 1970. There are 22 strikers featured on this disc. Their photos appear on the front as you turn the disc around. Details of the player, his club, the clubs nickname, their home ground at it's capacity are shown on the back.
